PHYSICAL FORM (below)
You will need to submit the following 4 pages to Brunswick Lacrosse: Physical Examination Form (pg 4), Medical Eligability Form (pg 5), Concussion Information Sheet-signature form (pg 10), Cardiac Arrest and Lindsay's Law signature form (pg 11).
This year we are only accepting the paper form of the physical. Physicals will no longer be uploaded to the web site. Please turn your physical into your coach as soon as possible, but no later than before your first game.

Joe is a Brunswick Lacrosse parent who has two lax players of his own. Sean is a 7th grader on our middle school team, and his brother Michael plays for the high school. He is sharing his artistic talent and has created some cool Brunswick lacrosse designs. He is now taking orders!
