PHYSICAL FORM INFORMATION
Please fill out THIS FORM for all youth athletes participating in the 2025 season by March 28th. We are no longer requiring the paper form of the physical to be turned in to coaches. Information contained in this form will be shared with the appropriate coaches, however, please ensure that you have a conversation with the coach regarding any health concern your athlete may have that could arise during a practice or game.

JoeRomano2's Artist Shop - fundraiser
Joe is a Brunswick Lacrosse parent who has two lax players of his own. Sean is a 7th grader on our middle school team, and his brother Michael plays for the high school. He is sharing his artistic talent and has created some cool Brunswick lacrosse designs. He is now taking orders!
